Perimenopause and Cholesterol: Why Your Lipid Panel May Not Tell the Whole Story

Your HDL rose. Your doctor told you your cholesterol was in great shape.
That could very well be the case. But it’s also one of the most misleading pieces of information a typical lipid panel can convey to a woman in her late 40s, because during perimenopause, HDL cholesterol levels tend to increase, even while their functionality decreases.
Cardiovascular disease is the number one killer of women. The menopausal transition is exactly when the risk profile changes for cardiovascular disease. Many women are warned about hot flashes and irregular periods, but very few are told that their lipid profiles and metabolic markers are also changing.
Here's what happens and what to measure.
HRT and Perimenopause Cholesterol
We get a lot of questions from women about whether taking hormones affects those rates. Estrogen is associated with shifts in lipid metabolism, and HRT perimenopause cholesterol effects can vary by the type, dose, and route you use. That’s a question to ask your prescriber, preferably with a baseline panel so you can see for yourself what’s going on in your body.

What Happens to Cholesterol During Menopause
How does perimenopause affect cholesterol
The strongest data supporting this notion come from SWAN, the Study of Women’s Health Across the Nation. This large, multiethnic study tracked women for several years through the menopause transition with multiple lipid measurements taken along the way, as opposed to one time.
SWAN also found that there was a rapid increase in total cholesterol, LDL cholesterol, and apolipoprotein B within approximately 1 year prior and 1 year after the final menstrual period, a perimenopause cholesterol spike that occurred over a relatively short time period. The perimenopause cholesterol levels remained high but did not increase as rapidly following the 1-year window before or after the final period.
The key here is that timing. If it were just about age, we'd see a continuous upward trend throughout the entire study. Instead, the rise is concentrated at the time of menopause, suggesting it's due to that change rather than aging.
Later SWAN research also showed that the degree of these perimenopause cholesterol changes was associated with indicators of subclinical atherosclerosis, in other words, not just figures on a lab report.
The HDL Problem
That's the finding that matters most, because it contradicts everything you've probably been told about cholesterol.
HDL is conventionally the "good" cholesterol, and more is conventionally better. Across the

menopause transition, HDL cholesterol and total HDL particle number often increase. On a standard lipid panel, that reads as an improvement.
However, when the SWAN researchers examined HDL subclasses and function, they found something quite different. During the 1-2 year periods centered around the final menstrual period:

Small HDL particles increased

HDL particle size decreased

Cholesterol efflux capacity per particle decreased
HDL’s primary function is to extract cholesterol from cells and tissues and return it to the liver, and a test of cholesterol efflux capacity measures how well HDL accomplishes that vital task. What researchers found was more HDL particles working less hard.
The researchers concluded: "HDL cholesterol increased overall across the transition, but the HDL subclasses and lipid content changed adversely, and function per particle decreased."
But for a woman seeing a lipid profile with an HDL reading of 65, it’s not apparent.
LDL Isn't Simple Either
In LDL particles, a similar change occurs. Postmenopausal women exhibit an increase in the number of small, dense, and ApoB-rich LDL particles and a decrease in the large, buoyant ones.
The reason this is important is that you can have a very "normal" LDL cholesterol level with a very high number of atherogenic particles. LDL cholesterol only reflects the amount of cholesterol contained within the LDL particles, not the actual number of particles. Because smaller particles carry less cholesterol each, a person will need a greater number of particles to deliver a certain amount of cholesterol to their tissues. Particle number, rather than cholesterol content, correlates better with risk.
What's Worth Measuring Instead
None of this means a standard lipid panel is useless. It means it's incomplete during a specific window when things are changing quickly.

Apolipoprotein B (ApoB) is the single most useful addition. Each atherogenic particle carries exactly one ApoB molecule, so measuring ApoB effectively counts particles directly. It's inexpensive, widely available, and increasingly recognized as a better risk marker than LDL cholesterol alone.
Small dense LDL measured directly, rather than inferred, captures the particle-size shift described above.
Lipoprotein(a) is worth measuring once in a lifetime. It's largely genetically determined, isn't captured on a standard panel, and is an independent risk factor a substantial minority of people carry without knowing.
Oxidized LDL reflects the modified particles most involved in early plaque formation.
Fasting insulin alongside glucose. Insulin resistance develops well before blood sugar rises, so glucose alone can look normal while the underlying picture is already changing.
hsCRP captures inflammatory activity, which also tends to rise during this transition.
Homocysteine relates to methylation capacity and vascular health, and B vitamin status, which is worth knowing during a stage when B vitamins matter for several reasons.
Getting These Tested
Most of these markers aren't included in a routine physical, and asking for them individually can be a frustrating process.
